Download Instructions :
  1. Click "Form" link.
  2. Save the form (*.doc file) to your hard drive.
  3. Either fill in the form from Word or print the form and than fill it in.
  4. Mail a printed copy of the form .
NOTE: If you do not get the Save/Open dialog box and would rather have the Save/Open dialog box appear, follow these steps below:
  1. Go to My Computer.
  2. On the Menu Bar , go to Tools.
  3. Select Folder Options from the Tools Menu.
  4. In the Folder Option dialog box select File Type tab..
  5. Once all file types have displayed, select DOC - Microsoft Word Document from the list.
  6. With DOC - Microsoft Word Document highlighted, click on the Advanced button.
  7. Check the "Confirm open after download" checkbox.
  8. Click OK then click Close.
